Once you choose a plan, the employee costs will be automatically deducted from your weekly paycheck. For questions regarding your eligibility please contact your local HR.

  • Medical Providers

    Your Medical Provider Network is Allied Benefit Systems for all locations.

  • Prescription Drug Plan

    CVS Caremark is our Prescription Drug Plan provider. Log into their portal here, to find a pharmacy or view prescription options.

  • Home Delivery

    With home delivery by CVS Caremark, they provide a convenient and affordable way for members to receive maintenance medications..

Services That Require Prequalification

The following require precertification, at least forty-eight (48) hours prior to receiving any of the following health care services. You are responsible to call or have your physician call:

  • ALL INPATIENT HOSPITALIZATION

  • CARDIAC REHABILITATION

  • MRI’S, PET SCANS, OR CAT SCANS

  • ALL OUTPATIENT SURGERY

You will pay $500 out of pocket if you or your physician do not pre-certify these services.
